Why does swelling last so long?
Swelling after rhinoplasty has two distinct layers. The first is the obvious oedema of the early days: the acute response of the tissues to surgery, which subsides quickly. The second is a fine layer of oedema beneath the skin; it is hard to see with the eye, but it affects the shape of the tip for months.
This second layer lasts particularly long where the skin is thick. The thicker the skin, the later it drapes over the new framework beneath. That is why two patients can have the same operation and quite different swelling timelines.
A realistic day-by-day timeline
Swelling and bruising at their peak. This does not mean healing is going badly; it is the top of the expected curve.
The splint comes off. You see your nose for the first time and it usually looks thicker than it is. It is too early to judge the result at this stage.
About 70% of the swelling has resolved. Most people look natural in daily life and it does not show in photographs.
The bridge and middle third settle. The tip is still a little full.
The oedema of the tip resolves. This is the final result. After revision surgery it can take up to 18 months.
What helps reduce swelling?
- Sleep with your head raised for the first week (two pillows).
- Apply cold around the eyes for the first 48 hours; do not press on the nose.
- Reduce salt; it increases swelling noticeably.
- Avoid strenuous exercise, hot showers and saunas for the first 3 weeks.
- Protect yourself from the sun; UV affects both swelling and scar quality.
- Do not massage or tape the nose unless your surgeon has told you to.
When should you be concerned?
Increasing swelling on one side, redness with local warmth, a temperature above 38 °C or bleeding that will not stop are not expected findings. In these cases contact us without waiting for your scheduled review.
